If you’d like me to see your patients, here’s what you need to know:

Thank you for taking the opportunity to see how you can refer your patients to me. I provide medical nutrition therapy in the following states AK, AZ, CA, CO, CT, FL, GA, HI, ID, IL, IN, MA, MD, MI, MS, NH, NJ, NY, OK, OR, PA, TX, UT, VT, VA, WA, WV, WI, WY.

What kind of patients do you see?

Most of the patients I serve are women suffering from endometriosis, uterine fibroids, and PCOS. I also see women who are pregnant or trying to conceive. I use evidence-based practices and a food-first approach to help support their medical conditions. I do not guarantee relief but many of my patients have noticed reduced severity in their symptoms.

If I cannot provide a patient with adequate care, I will refer them to a colleague who specializes in their concerns. Patients who typically get referred to my colleagues are patients that will do better with a sports dietitian, GI dietitian, plant-based dietitian, eating disorder dietitian, or renal dietitian. If you’re not sure a patient can see me, send them to me anyway and I will connect them to the appropriate provider if needed.

Do you accept insurance?

Yes! Through Nourish, I accept Aetna, Anthem, Blue Cross Blue Shield, Medicare of Texas. Via Berry Street, I accept Cigna and UnitedHealthcare. By other means, I also accept Medicare of Florida (not Medicare Advantage), Humana (HMOs require referrals), and Oscar Health Insurance.

Referrals

Please fax me a referral with diagnosis codes to my fax number 786-822-5533. Please also provide me with their H&P for continuity of care. The most common service codes are 97802 (initial assessment) and 97803 (follow-up assessments).
